For the Social Work Exam you want to make sure to remember the differences between, somewhat similar, DSM diagnoses. I always get confused with these two "labels". Just remember that <b>Conduct Disorder (CD)</b> is the extreme form of <b>Oppositional Defiant Disorder (ODD)</b>. Below is a list of childhood symptoms for each:</div>

Roberts is one of the founders of the crisis intervention model used by most clinicians today. Here are the steps defined:<br />
<br />
1) ASSESS for safety/suicidality and provide a brief biopsychosocial assessment<br />
2) Continue to develop RAPPORT by being empathetic and warm<br />
3) IDENTIFY PROBLEM/what lead to crisis and what other problems are concerning them at that moment<br />
4) Address FEELINGS and allow client to vent while validating and actively listening<br />
5) Identify ways they will COPE with situation and how they have coped with difficulties in past<br />
6) Create an ACTION PLAN with client, time to come to terms with crisis and attempt to resolve<br />
7) FOLLOW-UP with client via phone, in-office counseling, etc..<br />

Some of the easiest questions to miss on the Social Work Exam are those that make you apply a simple definition to a case example. Below are definitions and case examples to help you remember:<br />

Alcohol Abuse is not as extreme as Alcohol Dependence, although they both are harmful. For the full DSM-IV-TR criteria, <a href="http://www.ncbi.nlm.nih.gov/books/NBK44358/">click here</a><br />

<b>Example 1:</b><br />
For the past year, a 23 year old woman parties every weekend and will often wake up in unknown places. She is always late to work and fears that she will lose her job due to being too hung over all of the time. She recently was pulled over for a DUI but still continues to drive drunk.<br />
<br />
<i>This would be an example of Alcohol Abuse because this woman has continued substance use that resulted in failure to perform work, legal implications, and continued drinking even after she was aware of the consequences. The DSM states that only one of the impairments described above need to be in place in the last 12 months to be considered a Substance Abuser. </i><br />
<br />
<b>Example 2:</b><br />
A 40 year old man recently left his wife and children after a 3 day binge and losing his job. For the past year, this man has been drinking daily and has needed more and more to feel normal. He has attempted to stop many times but is unable to deal with the withdrawal symptoms. He hangs around his buddies more than his family because they understand him and don't mind his drinking.<br />
<br />
<i>This man is suffering from Alcohol Dependence. The DSM states that within a 12 month period someone needs to show 3 of the following: signs of tolerance, withdrawal, the need for more to achieve the desired effects, failed attempts to stop, wasted time on drinking activities, missing other life events in order to drink, continued use even after known harmful effects are present. </i><br />

<b>Please be aware that the DSM V (published May 2013) no longer distinguishes between alcohol abuse and dependence and instead places both terms together as part of the overall diagnosis of a Substance Use Disorder. Solution Focused Therapy is a great brief therapy method to use if you're trying to assist someone in a short amount of time. Below are 4 descriptions of the theory along with a helpful nemonic device to memorize for the exam: </div>

<span style="font-family: inherit;"><span style="background-color: white; color: #222222;">1) <b>Tricyclics</b>- These were one of the first anti-depressants used to treat clinical depression as they help provide a reuptake of Norepinephrine and Serotonin (SNRI) this allows for more of these two happy hormones to transmit more often in the brain. Unfortunately, there are many harmful side-effects of this drug and as a result, they are not prescribed as often. </span><br style="color: #222222;" /><br style="color: #222222;" /><span style="background-color: white; color: #222222;">2) <b>MAOIs</b>- Monoamine Oxidase Inhibitors- is a natural enzyme that breaks down serotonin, epinephrine, and dopamine. MAOIs block the effects of this enzyme. As a result, the levels of those neurotransmitters are higher. This drug is very dangerous when combined with other drugs and will therefore typically not be prescribed first.</span><br style="color: #222222;" /><br style="color: #222222;" /><span style="background-color: white; color: #222222;">3) <b>SSRIs</b>- Selective Serotonin Reuptake Inhibitor. These drugs will stop the uptake of serotonin and will allow that happy hormone to continue transmitting in the brain.These medications are the go-to for depression because they seem to be safer and have less side effects. </span><br style="color: #222222;" /><br style="color: #222222;" /><span style="background-color: white; color: #222222;">4) <b>NDRIs</b>- Neurotransmitters Norepinephrine Anddopamine. These drugs will make sure that both happy hormones, Norepinphrine and dopamine, reuptake in the brain. The Americans with Disabilities Act was brought into existence in 1990 (can you believe it wasn't until 1990!) and was based off the civil rights laws of the 1960's. A social worker learns very early on that we should meet the client or person we are serving where they are, which means to respect their ability to make their own choices. Social workers believe that people have the ability to live up to and develop their own potential. These are also themes found in the Humanistic Theory.</div>

<li class="li1">One of the founders of this theory was, existentialist, Jean-Paul Sartre. His famous quote was "Hell is other people" which, to me, means we have the capacity to make up our own minds about any situation and don't have to rely on opinions of others, we can color our world and attitude with our own minds.</li>
<li class="li1">Client-centered therapy falls into the Humanistic camp, because it allows the client to be the expert on their own lives. The role of the therapist is simply to empathize, while pointing out concerns and interests, in order to allow the person to realize and change on their own. </li>
<li class="li1">Gestalt Therapy also falls under this camp, because it also places the responsibility of the problem on the client themselves not on outside influences. It allows the client to be in the now and express themselves. I love the empty-chair technique where the client speaks to an empty chair which they imagine has a part of themselves (like their anger or their inner child) sitting there, it's a great way to separate parts of themselves and to develop insight.</li>

Murrary Bowen coined the idea of triangulation for the family therapy field. You may find this term on the social work exam when describing an emotional system where two people are unstable and will pull in another person in an unhealthy attempt to balance the stress. Families that struggle with mental illness, abuse, and alcoholism often are triangulated.<br />

As a medical social worker you may find two parents with a medically fragile child are triangulated with a healthy sibling. For example, a mother could have trouble communicating the stress of caring for a fragile child and will use another child to direct her frustration to the father who is not being supportive. She could also use the child as a messenger for communication instead of being direct and congruent. It's important to point out these examples of triangulation to clients so they can become aware and learn healthy ways to deal with stress and communicate congruently. <b>The <u>O</u>ral </b><b>stage</b> is from birth to age One. The <b>baby learns to suck</b> (a bottle, breast or <b>pacifier)</b> and develops healthy oral habits. If these habits don't develop it is said that an adult will bite their nails or over eat or smoke later in life, etc..<br />

<b>The <u>A</u>nal </b><b>stage</b> is from age 1-3 and it is when toddlers learn how to hold and release their urine and feces. They learn to <b>move away from diapers by using the toilet</b>. The way they are taught this important step can cause an adult to be normal or either too orderly or messy.<br />
<br />
<b>The <u>P</u>hallic</b> <b>stage</b> which is from ages 3-6. At this stage, is it said, that preschoolers develop oedipus and electra conflicts (sexual desire for boys to mother and girls to father). This causes the <b>child to take on characteristics of the same-sex parent</b>. The children will suppress this desire to not scare away the opposite sex parent and thus the superego is formed.<br />
<br />
<b>The <u>L</u>atency </b><b>stage</b> starts at 6 and end at age 11. During this stage the sexual instincts <b>hide away</b> and a child learns new social values with peers (<b>slumber party</b>).<br />
<br />
<b>The <u>G</u>enital </b><b>stage</b> starts in adolescence and extends through adulthood. The phallic stage reappears and if the earlier stages were developed successfully than this will lead to<b> marriage</b> and mature sexuality.<br />

Gregory Bateson (among many others) was one of the founding fathers of communication theory when it comes to social sciences. He coined the term: Double Bind- which originated from his studies with schizophrenic patients and their communication with family.<b> Double Bind </b>is like hitting someone every time you say I love you. There are two contradictory messages but it becomes a bind because the person cannot understand the contradiction.<br />

A lot of this type of communication is <b>Metacommunication </b>which means it is an interpretation of what is said and acted out.<br />

A <b>paradox</b> is a great method of communication to bring attention to the problems with metacommunication and double bind messages. A paradox is when a therapist prescribes the exact symptom that it causing the problem. So, for example, if a family continues to not communicate compliments to each other after doing a chore, then the therapist can tell the family to make sure to not say thanks or good job or any kind word after a family member finishes their chore. This is thought to give up control of that action and draw attention so that the negative remarks no longer serve their purpose.<br />

A medical social worker who works on the cancer unit in a children's hospital has grown very fond of a refugee family from Bengal who comes in with their 9 year old son every week for chemotherapy. The social worker has built a great rapport with the family and always stops by to either provide resources or just to check in with the family, along with a translator, every week. After a couple months, the social worker finds out from the 9 year old himself that his mother moved out of the home and it is now only the father that is caring for him and his two younger siblings aged 4 and 6. The social worker, who is well aware that the father already works two jobs to keep the family afloat, asks the father, through a translator, who is caring for the kids during the day while he is at work. He states that nobody is caring for them and that they care for themselves. WWSWD next?<br />

a) Educate the father on the different laws in the US and tell the father that he needs to hire a babysitter and make him provide a verbal agreement that he will.<br />

b) Don't say anything, because you've known the family a long time and know the kids are very mature for their ages and honor that it's part of their culture to care for each other.<br />

c) Educate the father on the different laws in the US and report this negligence to the Department of Family Services to investigate the home and provide resources.<br />

d) Provide father with a list of local daycare services.<br />
